
SWRS specializes in providing
to rehabilitation agencies social work assistance services that meet
Medicare’s regulatory requirements.
All SWRS social workers are licensed professionals, and some also have
additional certifications in geriatric care management, mental health,
rehabilitation, and addictions. These services will be available nationwide
through a direct inward-dial toll-free telephone number dedicated solely
to your company or directly to your local clinic as appropriate.
SWRS will provide social work services to include:
• Automated review of social services questionnaires, including scoring,
scanning, data entry and data evaluation of screening questionnaires
• Telephonic service to patients for information about social work services
• Computerized database information search to provide resources and information
related to standard informational requests for transportation and other assistance
• Telephonic or in-person problem assessment and evaluation, as clinically
indicated
• Referral to qualified community resources and providers, as appropriate
• Crisis intervention as appropriate, including the reporting of abuse
and neglect
• Consultation with family members and caregivers
• Screening, credentialing and contracting of qualified social workers
to advise and counsel on social problems that arise from a patient’s
illness or injury
• Maintenance of clinical records on all patient contacts and activities
in accordance with accepted professional standards and practices
• Follow-up telephone calls to ensure quality and patient satisfaction
SWRS will provide social work services to clinic staff to include:
• Consultation regarding troubled or distressed patients
• Staff education on methods to determine if a patient requires social
work services
• Customized education and training on topics related to patients troubled
by personal problems
SWRS will provide corporate services to include:
• Customized program design that meets or exceeds the requirements of the
regulations
• Review of organizational policies and procedures and development of social
work services policies and procedures
• Development of social work services work flows that reduce the demands
on clinic staff
• Development of evidenced-based screening tools
• Document management and database development
• Submission of standardized quarterly reports
