To meet the demands of a healthcare system experiencing rapid growth in the treatment of chronic illness and disabilities, the rehab industry is confronted with the responsibility of assessing risk, improving quality, reporting outcomes and containing costs. While innovative strategies and programs exist to empower consumers, foster health and wellness, and improve care for persons with chronic conditions, actually implementing and sustaining high-quality, cost-effective care management is a challenging task.

One solution used by SWRS in the measurement and quantification of patient-reported outcomes is the SF12 v2 Survey. The survey is evidenced based and scorable. It has supported over 1,400 payer and provider organization studies and 2,300 governmental and non-profit enterprise projects and provides:

• Scientifically validated measures of physical and mental health status
• Standardized metrics to allow for comparisons between treatments, outcomes, programs, patients, and populations


As a brief, reliable measure of overall health status, the SF-12v2 is the instrument of choice in large population health surveys and has been used extensively as a screening tool. Of particular note, SF-12v2 Health and Wellbeing Survey offers an evidence-based methodology to:


• Screen and stratify patients for treatment
• Predict those most likely to incur high future medical costs
• Measure and monitor treatment effectiveness over time, supporting best practices
• Improve patient-provider communications and treatment compliance


The SF-12v2 can be self-administered to persons ages 14 and older and usually takes two to three minutes to complete.It assesses across eight domains including physical functioning, role physical, bodily pain, general health, vitality, social functioning, role emotional, and mental health. A similar survey is available for pediatric patients.

Of note, the Agency for Healthcare Research and Quality has been using the SF-12 Health Survey, and most recently adopted the SF-12v2 Health Survey, to measure health status in the Medical Expenditure Panel Survey (MEPS).
